There were multiple bags with meat cubes in the freezer that were missing labels to identify what the meat was and where it was processed. The paper towel dispenser in the bathroom was empty. The health permit was expired.

Round Table Pizza, 2401 E. Orangeburg Ave. in Modesto, had five violations on Aug. 1 — two of them major. The cooked pizza in a hot-holding unit had a temperature of 132 degrees (must be 135 degrees). The hot-holding unit door was open and could not close. A bag of dough was stored on the floor. There was no sanitizer in the dishwasher. The facility lacked a valid permit to operate.
